Grant is going to get 300 carries to Gore's 230 or so because the 49ers year over year have shown the wonderful wisdom of taking the ball out of the hands of their best player.
Last season the two were separated by 30 points in ppr leagues, which is a relatively small margin. (my main league is 0.5 ppr and the gap was 20, in non-ppr even smaller...)
You state that Grant is losing TDs inside the 5. Yes, Grant plays in a much better offense - but the Packers don't ignore the running game for the sake of throwing the football. He still saw 11 TDs on the ground last season, which is more than Gore has had at any point in his entire career.
The only reason people have Grant ranked so low is that they believe his job is threatened while Gore has a stranglehold on his. Let's not forget that Grant played hurt the entire 2008 season and still managed over 300 carries and 1300 yards.
I am a huge fan of Gore and agree that he is the #5 dynasty RB, but unless the 49ers give him 275+ carries, his ceiling is limited. My comments about the two being close in value are more about how underrated owners have Grant and less about Gore.
